A Boston-based nonprofit organization with a mission to positively impact individuals experiencing homelessness, substance use disorder and mental health issues is seeking a Community Stabilization Case Manager to join our growing team!
Schedule:
- Monday - Friday - 40 hours per week. 8:30a-4:30p or 9a-5p
Job Summary:
- Provide a variety of stabilization services to Veterans who were homeless and placed into permanent housing in the greater Boston community.
- Work with the goals of the program and help to maintain stabilized housing.
- Work collaboratively with Community Stabilization program teams and with the larger Human Services teams to ensure best practice modalities for this Veteran population.
Required Qualifications:
- Bachelor's degree in Human Services or related field or 3 + years community case management experience.
- Experience working with a variety of behavioral health challenges, and/ or homeless population.
- Must understand and have ability to work with Harm Reduction, Motivational Interviewing and Trauma Informed Care best practices.
