Essential Responsibilities:

• Conduct outreach, assessments, and evaluations using a Housing First approach.
• Support individuals transitioning from shelter to permanent housing, addressing barriers to stability.
• Develop and implement participant-centered service plans, track progress, and maintain accurate case records.
• Educate and assist clients with daily living activities, appointments, and community resource navigation.
• Build and maintain relationships with landlords, property managers, and service providers to secure housing opportunities.
• Provide crisis intervention, mediation, and service coordination as needed.
• Participate in agency meetings, trainings, and collaborative efforts with internal and external partners.
