Essential Responsibilities:
- Conduct outreach, assessments, and evaluations using a Housing First approach.
- Support individuals transitioning from shelter to permanent housing, addressing barriers to stability.
- Develop and implement participant-centered service plans, track progress, and maintain accurate case records.
- Educate and assist clients with daily living activities, appointments, and community resource navigation.
- Build and maintain relationships with landlords, property managers, and service providers to secure housing opportunities.
- Provide crisis intervention, mediation, and service coordination as needed.
- Participate in agency meetings, trainings, and collaborative efforts with internal and external partners.
