Case Manager - East Boston

Supporting families experiencing homelessness in one of the region's largest shelter and stabilization programs, this role plays a key part in helping parents and children work toward safety, stability, and long‑term independence.

Requirements

• Bachelor's degree in Human or Social Services
• 2+ years of case management experience
• Background working with vulnerable or high‑needs populations

Job Description

• Provide intensive case management, assessment, counseling, and advocacy for families in shelter
• Build goal‑oriented, supportive relationships with families from intake through stabilization
• Collaborate with Housing Specialists, Behavioral Health Clinicians, and Family Partners
• Support families in developing rehousing and stabilization plans within a trauma‑informed framework
• Maintain accurate case files and documentation for internal and external reporting
• Deliver crisis intervention in partnership with behavioral health and leadership teams
• Serve as a positive role model and advocate for families' needs across systems

Schedule

• Full‑time, 40 hours per week
• Monday-Friday: 8am-4pm or 9am-5pm, with one late shift (10am-6pm or 11am-7pm)
• On‑call rotation: one weekend per month + one weekday per week
• Onsite role in East Boston
