This remote role requires an active RN license in Michigan and a Compact Nursing License that includes Florida RN privileges.
Job Description:
Conduct medication reconciliation and follow-up care for high-risk patients post-discharge.
Perform structured telephonic clinical assessments using motivational interviewing techniques.
Document patient encounters in real time using Care Convene while managing calls via a third-party phone system.
Communicate professionally with patients experiencing emotional or psychological barriers to care.
Maintain accurate and timely clinical documentation in electronic health records.
Escalate complex care management cases based on clinical criteria and protocols.
Maintain patient privacy with a quiet, secure home workspace and reliable internet connection.
Use Slack, Flex phone, and required tech tools; training and support provided during onboarding.
Adhere to HIPAA compliance, participate in ongoing quality evaluations, and work weekend shifts as needed.
Schedule: Monday - Friday, 10:00a - 6:00p EST
