Support Services Program<\/span><\/p>\n<\/p>\n<p><strong>Requirements<\/strong><\/p>\n<ul>\n<li>High school diploma or GED; valid MA driver&#8217;s license with 2+ years of driving experience<\/li>\n<li>Experience providing case management or direct support to individuals with complex needs<\/li>\n<li>Strong written communication, organizational skills, and proficiency with Microsoft Office<\/li>\n<li>Ability to work independently, manage crises, and maintain professional boundaries<\/li>\n<\/ul>\n<p><strong>Job Duties<\/strong><\/p>\n<ul>\n<li>Provide intensive case management to formerly chronically homeless tenants using a Housing First, harm\u2011reduction, and trauma\u2011informed approach<\/li>\n<li>Conduct community and office visits, complete intakes, and support housing transitions<\/li>\n<li>Create and update Housing Service Plans (HSPs), maintain case notes, and complete required documentation<\/li>\n<li>Support tenants with daily living skills, benefits applications, budgeting, and accessing health, legal, and social services<\/li>\n<li>Assist tenants with maintaining housing, resolving issues with property management, and addressing rent\u2011related concerns<\/li>\n<li>Advocate with community agencies, attend case conferences, and coordinate with housing authorities<\/li>\n<li>Provide crisis intervention, develop crisis plans, and support tenants with mental health or substance\u2011use needs<\/li>\n<\/ul>\n<p><strong>Additional Information<\/strong><\/p>\n<ul>\n<li><strong>Schedule:<\/strong> Monday-Friday, 8:00 AM-4:30 PM<\/li>\n<li><strong>Position Type:<\/strong> Temporary (6-8 weeks);
