This role provides hands-on case management focused on housing placement, homelessness prevention, community resource navigation, and ongoing stabilization.<\/p>\n<h2>Key Responsibilities<\/h2>\n<ul>\n<li>Conduct outreach, intake, assessments, and eligibility screenings for individuals seeking housing support.<\/li>\n<li>Help participants identify appropriate housing options and develop individualized plans focused on achieving and maintaining stable housing.<\/li>\n<li>Provide ongoing case management and connect participants with employment, legal, behavioral health, benefits, and other community resources.<\/li>\n<li>Assist participants with addressing barriers to housing, including credit, rental history, and legal concerns.<\/li>\n<li>Coordinate housing placement, homelessness prevention, and ongoing stabilization services within established program timelines.<\/li>\n<li>Build and maintain relationships with landlords, property managers, housing resources, and community partners.<\/li>\n<li>Maintain accurate case documentation, data entry, and reporting while participating in case conferences and team meetings.<\/li>\n<\/ul>\n<h2>Qualifications<\/h2>\n<ul>\n<li><strong>Bachelor&#8217;s degree in Human Services or a related field required.<\/strong><\/li>\n<li><strong>1-3+ years of relevant experience preferred<\/strong>, including case management, homeless services, housing programs, community resource navigation, or behavioral health.<\/li>\n<li>Familiarity with <strong>Motivational Interviewing, Harm Reduction, and Trauma-Informed Care<\/strong>.<\/li>\n<li>Valid driver&#8217;s license and ability to travel locally throughout the Greater Boston area.<\/li>\n<li>Strong communication, organizational, and computer skills.<\/li>\n<li>Comfortable working with diverse individuals with complex service needs.<\/li>\n<\/ul>\n<h2>Schedule &amp;
