This role focuses on building supportive relationships, connecting individuals with appropriate resources, promoting housing stability, and helping participants work toward their individual goals.<\/p>\n<h2>Key Responsibilities<\/h2>\n<ul>\n<li>Manage an assigned caseload and connect participants with services and community resources that align with their individual needs and goals.<\/li>\n<li>Provide supportive counseling using evidence-based approaches including Motivational Interviewing, Harm Reduction, Relapse Prevention, and Trauma-Informed Care.<\/li>\n<li>Complete biopsychosocial assessments, individualized service plans, discharge plans, progress notes, and other required documentation.<\/li>\n<li>Work collaboratively with participants toward permanent housing, increased income and benefits, greater stability, and improved self-determination.<\/li>\n<li>Respond to crisis situations and provide appropriate intervention and de-escalation while helping maintain a safe environment.<\/li>\n<li>Coordinate community-based referrals and provide appointment accompaniment when needed to support continuity of care and independent living.<\/li>\n<li>Collaborate with clinical and operational teams while participating in case conferences, program meetings, and other required meetings.<\/li>\n<\/ul>\n<h2>Qualifications<\/h2>\n<ul>\n<li><strong>Bachelor&#8217;s degree in a human services-related field<\/strong&gt;; relevant experience may be considered in lieu of a degree.<\/li>\n<li><strong>3+ years of experience<\/strong> in case management and working with individuals experiencing substance use disorders, mental health concerns, or other complex needs.<\/li>\n<li>Knowledge of <strong>Harm Reduction, Motivational Interviewing, Relapse Prevention, Trauma-Informed Care, and other evidence-based service models<\/strong>.<\/li>\n<li>Experience with <strong>crisis intervention and de-escalation<\/strong>, including the ability to exercise sound clinical judgment and maintain professional boundaries.<\/li>\n<li>Strong written and verbal communication skills, including the ability to complete clear progress notes and service plans.<\/li>\n<li>Strong computer and electronic recordkeeping skills.<\/li>\n<li>LADC or CADC credential is a plus.<\/li>\n<\/ul>\n<h2>Schedule &amp;
