The RN will handle inbound patient calls, complete medication reconciliation and post-discharge assessments, address immediate clinical questions, and document information needed to close quality care gaps.<\/p>\n<h3>Requirements<\/h3>\n<ul>\n<li>Active <strong>Registered Nurse (RN) license in Michigan<\/strong><\/li>\n<li><strong>3-5 years of nursing experience<\/strong>, with bedside or direct clinical experience highly preferred.<\/li>\n<li>Prior <strong>inbound call center experience<\/strong> with strong patient-facing telephone and customer service skills.<\/li>\n<li>Strong technical skills and ability to multitask in a fast-paced environment; <strong>Epic or Cerner EMR experience preferred<\/strong>.<\/li>\n<\/ul>\n<h3>Job Duties<\/h3>\n<ul>\n<li>Receive inbound warm transfers from patients following hospital or emergency department discharge and provide appropriate RN support.<\/li>\n<li>Conduct telephonic <strong>medication reconciliation (MRP)<\/strong> and <strong>7-day post-ED follow-up assessments (FMC)<\/strong> to support HEDIS care gap requirements.<\/li>\n<li>Review post-discharge medication orders, clarify dosing schedules, address potential medication interactions, and confirm patient understanding.<\/li>\n<li>Assess patient symptoms and provide appropriate clinical clarification during the call.<\/li>\n<li>Accurately document assessments and required quality data within the patient&#8217;s medical record.<\/li>\n<li>Resolve immediate post-discharge clinical questions within a high-volume, technology-driven call queue.<\/li>\n<li>Provide focused, episode-based nursing support without responsibility for ongoing care plans or outpatient referral management.<\/li>\n<\/ul>\n<h3>Additional Information<\/h3>\n<ul>\n<li><strong>Schedule:<\/strong> Part-time, 24 guaranteed hours per week, with potential for up to 8 additional hours.<\/li>\n<li><strong>Primary Hours:<\/strong> Monday-Wednesday, 10:00 AM-6:00 PM ET, with potential to transition to 11:00 AM-7:00 PM ET.<\/li>\n<li><strong>Additional Coverage:<\/strong> Flexibility to provide Thursday\/Friday coverage as needed.<\/li>\n<li><strong>Location:<\/strong> Remote;
