Education & Licensure
- PharmD from an accredited School of Pharmacy
- Active, unrestricted Pharmacist license (State Board of Pharmacy) Texas
- Sterile Compounding / IV Certified
Responsibilities:
- Assess patients for home infusion home care services.
- Develop and monitor medication and nutrition therapy plans.
- Review labs and adjust treatment recommendations as needed.
- Educate patients, caregivers, and healthcare staff.
- Prepare, verify, and dispense medications in compliance with regulations.
- Manage infusion equipment and support quality assurance programs.
- Respond to drug information requests and oversee pharmacy operations
