We are seeking a full-time Housing Case Manager to provide community-based support to individuals and families transitioning out of homelessness. This role is rooted in a Housing First approach and involves working directly with participants in their homes and communities to promote housing stability and self-sufficiency.
Key Responsibilities:
- Provide outreach, assessment, and ongoing case management
- Develop and implement individualized service plans
- Facilitate housing placement and retention
- Coordinate with service providers, landlords, and community partners
- Maintain accurate and timely documentation
Requirements:
- Bachelor’s degree preferred or 2+ years of relevant experience
- Strong communication and organizational skills
- Empathy and professionalism in working with vulnerable populations
- Ability to transport clients in a personal vehicle
Schedule: Monday-Friday, flexible hours between 8:30 AM – 5:00 PM
This position is ideal for someone passionate about housing stability and interested in working with specialized populations, including veterans. Transferable skills welcomed-training provided for the right candidate.
