A Boston-based nonprofit with a mission of advocating for families experiencing homelessness in the Greater Boston area is seeking a Supportive Housing Case Manager to join their growing team.
OVERVIEW :
- Provides case management to formerly homeless individuals and families and provides assistance and connection to services.
- Supports clients in remaining stably housed in apartments in scattered site model, or in congregate locations that are community based Using evidence based practices
- The Case Manager works with clients to develop and monitor individual service plans (ISP) which include educational, vocational, medical, life skills and housing goals.
- Assist program participants in gaining access to resources.
QUALIFICATIONS :
- Associate’s Degree with 2+ years experience preferred
- Experience with crisis intervention regarding issues such as mental health and substance use disorders
- Sensitivity to the needs of, people currently and formerly experiencing homelessness, low income and diverse populations;
- Valid MA driver’s license with access to independent mode of transportation (mileage reimbursement as applicable)
