Summary:
This role supports individuals and families at risk of homelessness by connecting them with housing resources, financial assistance, and community supports. The position involves direct client work, resource coordination, and ongoing case management to promote long-term housing stability.
Key Responsibilities:

Provide brief counseling and referrals for issues like financial instability, housing needs, and legal or health concerns

Administer homelessness prevention funds (e.g., RAFT), including eligibility screening and paperwork

Support clients in housing searches and move-in processes

Maintain regular contact with clients, complete service plans and assessments

Coordinate housing workshops and track participant progress

Submit weekly reports and maintain accurate documentation in client systems

Participate in trainings and team meetings

Assist with operational needs and other duties as assigned
Requirements:

High school diploma and 1-2 years of case management experience (preferably with homeless or low-income populations)

Valid driver’s license and access to a vehicle for community visits
Schedule:
Full-time, (office and community-based)
Mon-Wed: 8:45 AM-4:15 PM
Thurs: 8:45 AM-8:00 PM
Fri: 8:45 AM-12:00 PM
