Housing Case Manager
Location: Jamaica Plain, MA
Organization Type: Community‑Based Housing & Support Services Program
Requirements
- High school diploma or GED; valid MA driver’s license with 2+ years of driving experience
- Experience providing case management or direct support to individuals with complex needs
- Strong written communication, organizational skills, and proficiency with Microsoft Office
- Ability to work independently, manage crises, and maintain professional boundaries
Job Duties
- Provide intensive case management to formerly chronically homeless tenants using a Housing First, harm‑reduction, and trauma‑informed approach
- Conduct community and office visits, complete intakes, and support housing transitions
- Create and update Housing Service Plans (HSPs), maintain case notes, and complete required documentation
- Support tenants with daily living skills, benefits applications, budgeting, and accessing health, legal, and social services
- Assist tenants with maintaining housing, resolving issues with property management, and addressing rent‑related concerns
- Advocate with community agencies, attend case conferences, and coordinate with housing authorities
- Provide crisis intervention, develop crisis plans, and support tenants with mental health or substance‑use needs

- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Position Type: Temporary (6-8 weeks); potential to extend
- Pay: $24.50 per hour
