Clinical Case Manager
Location: Boston, Massachusetts
Organization Type: Community-Based Human Services Organization
Schedule: Full-Time | Monday-Friday, 7:00a – 3:00p EST
Pay Rate: Up to $26.50/hour
Position Overview
We are seeking a Clinical Case Manager to provide case management and supportive counseling to individuals experiencing homelessness and co-occurring mental health and/or substance use concerns. This role focuses on building supportive relationships, connecting individuals with appropriate resources, promoting housing stability, and helping participants work toward their individual goals.
Key Responsibilities
- Manage an assigned caseload and connect participants with services and community resources that align with their individual needs and goals.
- Provide supportive counseling using evidence-based approaches including Motivational Interviewing, Harm Reduction, Relapse Prevention, and Trauma-Informed Care.
- Complete biopsychosocial assessments, individualized service plans, discharge plans, progress notes, and other required documentation.
- Work collaboratively with participants toward permanent housing, increased income and benefits, greater stability, and improved self-determination.
- Respond to crisis situations and provide appropriate intervention and de-escalation while helping maintain a safe environment.
- Coordinate community-based referrals and provide appointment accompaniment when needed to support continuity of care and independent living.
- Collaborate with clinical and operational teams while participating in case conferences, program meetings, and other required meetings.
Qualifications
- Bachelor’s degree in a human services-related field
- 3+ years of experience in case management and working with individuals experiencing substance use disorders, mental health concerns, or other complex needs.
- Knowledge of Harm Reduction, Motivational Interviewing, Relapse Prevention, Trauma-Informed Care, and other evidence-based service models.
- Experience with crisis intervention and de-escalation, including the ability to exercise sound clinical judgment and maintain professional boundaries.
- Strong written and verbal communication skills, including the ability to complete clear progress notes and service plans.
- Strong computer and electronic recordkeeping skills.
- LADC or CADC credential is a plus.
