The Case Manager plays a key role in supporting families residing in a temporary housing program. Working alongside a multidisciplinary team that may include housing, clinical, and family support professionals, this individual helps participants identify goals, access resources, and make progress toward stable, long-term housing.
From intake through transition, the Case Manager provides consistent guidance, advocacy, and follow-up to ensure each family receives coordinated care tailored to their needs. The position emphasizes trauma-informed engagement, goal planning, and active collaboration across service providers.
In addition to direct support, the Case Manager maintains thorough and confidential documentation in accordance with program standards and funding requirements. This role also assists with crisis response, problem-solving, and connecting participants to behavioral health or other supportive services as needed.
