Case Manager – East Boston
Supporting families experiencing homelessness in one of the region’s largest shelter and stabilization programs, this role plays a key part in helping parents and children work toward safety, stability, and long‑term independence.
Requirements
- Bachelor’s degree in Human or Social Services
- 2+ years of case management experience
- Background working with vulnerable or high‑needs populations
Job Description
- Provide intensive case management, assessment, counseling, and advocacy for families in shelter
- Build goal‑oriented, supportive relationships with families from intake through stabilization
- Collaborate with Housing Specialists, Behavioral Health Clinicians, and Family Partners
- Support families in developing rehousing and stabilization plans within a trauma‑informed framework
- Maintain accurate case files and documentation for internal and external reporting
- Deliver crisis intervention in partnership with behavioral health and leadership teams
- Serve as a positive role model and advocate for families’ needs across systems
Schedule
- Full‑time, 40 hours per week
- Monday-Friday: 8am-4pm or 9am-5pm, with one late shift (10am-6pm or 11am-7pm)
- On‑call rotation: one weekend per month + one weekday per week
- Onsite role in East Boston
