Clinical Success Manager, RN

Location: New York, NY | Onsite
Organization Type: Healthcare technology organization focused on senior care coordination and patient advocacy

This is an early-stage opportunity to join a growing healthcare startup as one of the first members of its patient care delivery team. The Patient Advocate will work directly with older adults and their families while helping build the workflows, processes, scripts, and escalation procedures that will shape how the organization delivers care as it scales.

The role combines hands-on patient advocacy with the opportunity to help build a care model from the ground up. The ideal candidate is comfortable operating in a startup environment, taking ownership beyond a traditional job description, and turning real-world patient experiences into standardized processes that future team members can follow.

Requirements

  • Current, unrestricted RN, NP, or PA license in good standing in NY
  • 3+ years of patient-facing experience working with older adults in nursing, care coordination, navigation, case management, or a related setting
  • Strong communication, relationship-building, and follow-through skills when working with patients and families
  • Comfortable using AI-enabled tools and technology to support documentation, tracking, and care coordination

Job Duties

  • Serve as the primary point of contact for assigned older adults and their families, coordinating care from start to finish
  • Manage post-hospital follow-ups, referrals, appointments, medication reconciliation, and other ongoing care needs
  • Coordinate communication across healthcare providers, pharmacies, insurers, family members, and other members of the patient’s care network
  • Assist with insurance denials, appeals, and other barriers that may prevent patients from accessing needed care
  • Participate in or support patient appointments as needed and ensure appropriate follow-up actions are completed
  • Maintain accurate, up-to-date patient information and review technology-generated documentation and care tracking for accuracy
  • Develop and document standardized workflows, scripts, escalation procedures, and handoff processes to support future team growth and consistency

Additional Information

  • Schedule: Monday through Friday, 40 hours per week; standard office hours with some flexibility
  • Work Arrangement: Onsite in New York, NY
  • Compensation: $95,000-$135,000 annual base salary, plus equity
  • Benefits: Healthcare benefits provided
