Stabilization Case Manager
Location: Boston, Massachusetts

Schedule: Full-Time | Monday-Friday
Pay Range: $24-$26/hour
Position Overview
We are seeking a Stabilization Case Manager to support individuals and families who are experiencing or at risk of homelessness. This role provides hands-on case management focused on housing placement, homelessness prevention, community resource navigation, and ongoing stabilization.
Key Responsibilities
- Conduct outreach, intake, assessments, and eligibility screenings for individuals seeking housing support.
- Help participants identify appropriate housing options and develop individualized plans focused on achieving and maintaining stable housing.
- Provide ongoing case management and connect participants with employment, legal, behavioral health, benefits, and other community resources.
- Assist participants with addressing barriers to housing, including credit, rental history, and legal concerns.
- Coordinate housing placement, homelessness prevention, and ongoing stabilization services within established program timelines.
- Build and maintain relationships with landlords, property managers, housing resources, and community partners.
- Maintain accurate case documentation, data entry, and reporting while participating in case conferences and team meetings.
Qualifications
- Bachelor’s degree in Human Services or a related field required.
- 1-3+ years of relevant experience preferred, including case management, homeless services, housing programs, community resource navigation, or behavioral health.
- Familiarity with Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
- Valid driver’s license and ability to travel locally throughout the Greater Boston area.
- Strong communication, organizational, and computer skills.
- Comfortable working with diverse individuals with complex service needs.
Schedule & Additional Information
- Schedule: Monday-Friday, 9:00 AM-5:00 PM
- Pay Range: $24-$26/hour
- Location: Boston, Massachusetts
- Local travel throughout the surrounding service area is required.
- Position focuses on housing placement, homelessness prevention, case management, and longer-term housing stabilization.
